Information that we require when submitting a new fault request

1. Enter a summary for the problem

2. Enter a detailed description of the fault.

3. Please enter as much information as possible, including any error messages

4. Choose Category for request from drop down menu

5. Click the Choose File option to attach IT Fault log form Click on Submit Request to complete
